of our staff.Job Profile SummaryUnder the direction of the PACE
Center Manager, the Activity Coordinator will be responsible for
oversight of the Day Center Activities and ensuring the Activities
Team carries out the activity program. Responsibilities include
coordinating, supervising, and implementing all day center
activities and social programs for frail elderly and functionally
impaired older adults at the Day Center in an integrated program
setting. All PACE policies and procedures will be followed at all
times. The Activity Coordinator will be responsible for the
development and implementation of daily, monthly, and special
events activities, etc.Clinic Details

- Plans a comprehensive program tailored to the needs, interests,
and limitations of frail elderly persons, to promote optimum
health, independence and self-care; increase or maintain social
skills and self-esteem.
- Evaluate all PACE participants' functional status in
relationship to participation in recreational activities.
Establishes a realistic goal and set up a treatment plan to utilize
recreation therapy techniques. Re-evaluate PACE patients on a
regular basis.
- Works in collaboration with other staff members (social
workers, physical therapist, occupational therapist, CNAs, etc.) to
develop care plans that meet participant's needs, resources, and
interests.
- Works in collaboration with PACE IDT to determine the Plan of
Care for PACE participant/member.
- Completes required progress notes and six-month reassessments
for PACE participants.
- Supervise and coordinate Program Aids, Activity Aide, Community
Service providers and volunteers who are assigned to the activities
program by instruction, observation and open discussion; make
recommendations for evaluations to the Center Manager.
- Develop and posts a monthly calendar of events.
- Assists with in-service training and planning of community
events.
- Develops and work within an Activity Program budget that meets
the Center Manager's approval; submits all requests for purchasing
in.
- Establishes an activity volunteer program and work with
community organizations to support and enhance the Activity
Program.
- Keeps abreast of all regulations and standards to ensure
compliance with orders or directives issues by duly constituted
government/regulatory agencies (PACE, CMS, DHCS, etc.)
- Meets the administrative requirements including capturing data
and preparing and submitting required reports.
- Be proficient in and become user of EHR and responsible for
data entry of all required documentation on a daily basis.
Responsible for developing specific care plan that meet
participants needs, interests and skills to enhance participant's
health and wellbeing.Requirements:
- Have more than one year of experience in a social or
recreational program within the past five years, preferably in a
senior care setting.
- Be a certified activity coordinator or equivalent
education.
- Flexibility, initiative, creativity and excellent
organizational and time management skills.
- Prefer bilingual (English/Spanish).Knowledge, Skills, and
Abilities
- Knowledge of psychosocial, behavioral, and family needs of the
elderly population.
- Proven ability to work in an interdisciplinary team.
- Ability to work effectively and harmoniously with the staff,
the elderly, and providers of services, public, and private
agencies.
- Energetic, dependable, resourceful, and flexible.
- Effective oral and written communication skills.

hired at Family Health Centers of San Diego, your final base salary
compensation will be determined based on factors such as geographic
location, jurisdictional requirements, skills, education, and/or
experience. In addition to these factors - we believe in the
importance of pay equity and consider internal equity of our
current team members as a part of any final offer. Please keep in
mind that the range mentioned above is what we reasonably expect to
pay for the role. Hiring at the maximum of the range would not be
typical in order to allow for future and continued salary growth.
